If you are aged 75 or over and eligible for Medicare, you may be able to have a yearly health assessment. This looks at your overall health, medicines, falls risk, memory, mood, continence, vaccines and support at home.
GP Chronic Condition Management Plans (GPCCMPs)
If you have a condition that has lasted, or is likely to last, 6 months or more, your GP can help create a care plan. This may include referrals to allied health, such as a physiotherapist, dietitian or podiatrist.
